Membership Cancellation Policy:
1 // If you need to cancel for now, but plan to reactivate your membership in the future (and we hope you do) membership rates in effect at the time of reactivation will be applicable as membership rates are subject to change.
2 // If you would like to put your membership on hold instead of canceling, you can submit a Membership Hold Request. For this option, you will need to provide an end date for your hold. It cannot be indefinite. If you decide to cancel after a hold, you will need to make a final payment prior to cancellation. The hold is for returning members not to put off cancellation.
3 // If you're considering cancelling, but are on the fence, please reach out to Lesley Price or a coach you work closely with. If you’re sure you’d like to cancel, just complete and submit the form. This will serve as your written cancellation notice as required by your membership agreement.
4 // If there is a membership payment scheduled within the next 30 days, it will be charged as scheduled. No refunds/credits. All payments are non-refundable.

The member cancellation policy is located either in your signed waiver, you were sent an email with notification of this policy, and was verbally reiterated in your on-boarding session. To proceed with membership cancellation, please fill out and sign the online form below. Thank you again and it's been a pleasure to serve you!

By submitting this cancelation form, you are opting to discontinue your membership as it is currently charging instead of continuing to pay your membership. Signing this form allows our staff to deactivate your account following receipt of this form and completion of the scheduled payment on your account. Without completion of this form, your membership will either continue charge or the account will be considered delinquent.
I understand if I am currently enrolled in a monthly auto-renew at Elysium Training, the remaining balance on my program will become due and auto processed on your scheduled monthly payment if it’s within 30 days of my requested cancelation date.
If payment is not able to be auto processed, then an invoice will be sent via email with your outstanding balance and payment is due upon receipt.
If payment due is not completed, the account will be deactivated but considered delinquent and balance will need to be paid upon return.
